NEMT insurance providers
National Interstate, RLI's E&S auto unit, Draco (part of the Amwins Underwriting division) and Progressive Commercial each publish an NEMT or paratransit appetite on their own pages; Progressive says it writes NEMT in select states. RLI says to get its quote through a wholesale broker, and Draco's program is distributed through a select group of retail brokers. NEMT insurance itself is commercial auto coverage for paid rides to medical care. Which market fits depends on your state, fleet size, years in business and loss history, and on the limits your broker or facility contract names.

What is NEMT insurance?
NEMT is non-emergency medical transportation, and for Medicaid members it is the state's job. 42 CFR 431.53 says a state plan must "Specify that the Medicaid agency will ensure necessary transportation for beneficiaries to and from providers," and 42 CFR 440.170 lets the state run a brokerage program for members "who need access to medical care or services, and have no other means of transportation," with rides that "include wheelchair vans, taxis, stretcher cars, bus passes and tickets." A broker the state contracts with under that option must have oversight procedures to "ensure that transportation is timely and that transport personnel are licensed, qualified, competent, and courteous."
NEMT insurance is the commercial auto policy written for that work, plus the coverages around it. RLI calls it "specialized commercial auto insurance for businesses that transport passengers to medical appointments, healthcare services and other destinations when emergency medical transportation is not required." A personal policy does not fit: GEICO lists "Transporting goods or people for a fee" first among the "clear signs you need commercial coverage."
Which insurance companies write NEMT insurance?
These four markets publish an NEMT or paratransit appetite on their own pages, read on 2026-10-03. A row here is a place to submit, not a promise of terms.
The name on the quote is not always the insurer on the policy. RLI writes NEMT on RLI Insurance Company admitted paper in Florida and Illinois and on Mt. Hawley Insurance Company non-admitted paper elsewhere. Draco's page describes its carrier only by rating and does not name it, so ask whose paper a Draco quote is on. National Interstate also runs Mobility, which it calls "the first homogenous group rental captive designed just for NEMT and paratransit operators."

Does GEICO or Progressive insure NEMT vehicles?
Progressive does, in some states. Its NEMT page says "We write non-emergency medical transportation insurance in select states" and does not say which. Its livery product as a whole is offered in 40 listed states, and in Alaska, Florida, Illinois, Nevada and New Jersey a buyer "might be eligible for a for-hire livery insurance quote through a Progressive Commercial specialty agent." Ask about NEMT in your state by name.
GEICO publishes no NEMT appetite that we found. Its commercial van page offers coverage for "cargo, passenger, and wheelchair vans," and its commercial auto page counts "transportation and delivery services" as eligible, but no GEICO page we checked names NEMT, paratransit or paid passenger transport. Ask GEICO whether a for-hire NEMT account qualifies, in writing, before counting on a quote.
What insurance does an NEMT business need?
The core is commercial auto liability. The ISO business auto form pays what an insured "legally must pay as damages" for injury or damage "resulting from the ownership, maintenance or use of a covered 'auto'": the rider in your van and the people in the other car. National Interstate lists physical damage, general liability and property alongside auto liability for NEMT and paratransit operators. Progressive's NEMT page adds that "Businesses, agencies, and government programs that you contract with may also have specific insurance requirements to adhere to," so build the policy to the written contract.
- Auto liability at the limit the contract names; Draco's program lists $1M.
- Physical damage on owned vans, which the form extends to loss "to a covered 'auto' or its equipment."
- General liability for exposures away from the van; the CGL form excludes injury "arising out of the ownership, maintenance, use or entrustment to others" of any auto an insured owns or operates.
- Workers' compensation for drivers; the auto form excludes injury to your own "employee" arising out of employment.
- Hired auto for vans you lease, rent or borrow, which Progressive says "Extends your protection to leased, rented, or borrowed vehicles"; RLI writes hired and non-owned liability "standalone or in addition to fleet coverage."
- Professional liability where the contract asks; Draco says "GL/PL are also available via another carrier."
How is a wheelchair van insured?
A wheelchair van that carries paying riders goes on a commercial policy like any NEMT vehicle; the conversion is what needs care. Under the ISO business auto form the physical damage limit is the lesser of "The actual cash value of the damaged or stolen property" and "The cost of repairing or replacing" it with like kind and quality. Progressive says "Insurance companies may have specific guidelines for vehicles equipped with wheelchair lifts or ramps."
RLI says "Any modifications or permanently attached equipment must be disclosed on the submission," and its vehicle schedule asks for seating capacity, modifications and a stated amount per unit. A stated amount is a ceiling, not a promise: the ISO endorsement warns it "is not necessarily the amount you will receive at the time of 'loss'."
Price: no published per-van figure from any of the four markets. RLI says its NEMT minimum premium is $50,000 (page read 2026-10-03); Draco lists its minimum premium as "Varies." Progressive says "There isn't a one-size-fits-all livery insurance cost," and its NEMT page counts the vehicle's value, including equipment, among the factors.
Should I buy NEMT insurance through a broker, an agent or direct?
Through someone who can reach the markets that write it. RLI tells buyers to "ask your wholesale broker for a quote from RLI"; Draco's program is "distributed through a select group of retail brokers"; National Interstate works "with brokers across the country." Progressive is the one you can start yourself, in the states where it writes NEMT: "Get an online quote in minutes or reach out to an agent." Before you send loss runs to an agent, ask which of these markets it can submit to. RLI and Draco each publish what a submission needs; the list below follows them.
Then ask which insurer is on the policy and whether it is admitted in your state. Where Utah is the home state, Utah Code 31A-15-103 requires a policy from a nonadmitted insurer to carry a statement that the insurer "is not fully subject to regulation by the Utah insurance commissioner" and that "This policy receives no protection from any of the guaranty associations" set up under Utah's insurance code. If your broker contract limits which insurers it accepts, check before you bind.
Is NEMT insurance different in California, Florida, Texas or another state?
Which markets will write you changes by state, and so do the state's rules. Progressive's livery page says "The rules around livery insurance differ from state to state. Some may require higher limits or additional coverages." In Utah, 31A-22-305(5)(b) requires a business that accepts payment for transporting people to carry uninsured motorist coverage of at least $25,000 per person and $500,000 per accident. Utah Medicaid tells members who need door-to-door rides that they "may be eligible for ModivCare services," and says nothing about provider insurance, so ask the broker for its written requirement.
Below is where the three markets with a state list say they write. "Not excluded" is not a yes: RLI says "Each risk is reviewed individually." The Progressive column is its livery list; Progressive does not say in which states it writes NEMT.

Which NEMT insurance company is best?
Best means the market that will take your account and meet your contract. Each publishes its own lines:
- New or established: Draco lists three years in business; RLI says it "can consider new NEMT and paratransit ventures."
- Size: Draco lists a minimum of five units; RLI says its minimum premium is $50,000 (page read 2026-10-03).
- Vehicles: RLI lists "Vehicles with 15+ passenger capacity including the driver" as ineligible.
- History: RLI says its solutions are based on factors including a non-renewal or cancellation notice, claim frequency or severity, and new-venture status.
What to send for an NEMT quote
The first six follow RLI's and Draco's published submission lists. The last is there because Progressive's NEMT page says the businesses, agencies and government programs you contract with "may also have specific insurance requirements."

What is the difference between NEMT and paratransit?
RLI's answer: NEMT "primarily provides non-emergency transportation to and from medical appointments and healthcare services," while paratransit "generally provides accessible transportation for individuals who cannot independently use conventional public transportation." RLI adds that some businesses provide both.
